Collective Health is the administrator for Petco’s Enhanced PPO, Value PPO and HSA medical plans. Collective Health utilizes the Blue Shield medical provider network.
With Collective Health’s unique platform, you will be able to view your medical, pharmacy and dental claims all in one place (excluding Kaiser plans and the dental HMO). You can work online to manage your healthcare claims and access tools that help you find care and compare costs for services and procedures in your geographic area. You can also speak to a Member Advocate online or by phone to ask questions about your coverage and for help submitting claims. Collective Health makes it simple for you to make the most effective decisions for your health and your wallet.

You can learn about the medical plans Collective Health administers at https://join.collectivehealth.com/petco.
After you enroll, use the Subscriber ID on your combined Medical/Pharmacy ID card to register on the Collective Health website at https://my.collectivehealth.com.
Once you are registered, you can view, print and request ID cards, submit medical claims or track your claims, view medical benefits information and much more.
